About this Course

Understanding Crisis and Trauma

Defining Crisis and Critical Incidents

  • A crisis is defined as a disruption of an individual's or group's normal state, often overwhelming coping mechanisms. This course will differentiate between everyday stressors and critical incidents.
  • Critical incidents involve events that are sudden, unexpected, and intensely negative, potentially leading to significant psychological distress. Examples: Natural disasters, acts of violence, serious accidents, or the sudden death of a colleague.
  • Distinguish between acute, delayed, and cumulative stress reactions. Acute stress occurs immediately following a crisis, delayed stress manifests later, and cumulative stress builds over time from multiple exposures.

The Neurobiology of Trauma

  • Explore the neurological impacts of trauma, including the role of the amygdala (emotional processing), hippocampus (memory consolidation), and prefrontal cortex (executive functions). Understanding how these areas are affected informs intervention strategies.
  • Discuss the stress response system (hypothalamic-pituitary-adrenal axis - HPA axis) and its dysregulation following trauma.
  • Examine the concept of 'window of tolerance' and how trauma can narrow this window, leading to hyper- or hypo-arousal states. Learn techniques to help individuals regulate their nervous system and return to their optimal zone of functioning.

Psychological Trauma

  • Explain the concept of psychological trauma as an emotional response to a distressing event that overwhelms an individual's ability to cope.
  • Differentiate between single-incident trauma and complex trauma. Single-incident trauma involves a single, isolated event, while complex trauma results from chronic or repeated exposure to traumatic events.
  • Describe common psychological reactions to trauma: anxiety, depression, anger, guilt, shame, and dissociation.

Core Principles of Crisis Incident Stress Management (CISM)

The SAFER Model

  • Detail the SAFER model: Stabilize (ensuring safety and security), Acknowledge (validating feelings), Facilitate (encouraging adaptive coping), Encourage (promoting self-efficacy), and Refer (connecting individuals with appropriate resources).
  • Explain how to apply the SAFER model in various crisis situations, tailoring the approach to the specific needs of the individuals involved.
  • Understand how SAFER can be used in individual and group interventions.

Ethical Considerations in CISM

  • Discuss ethical principles in crisis intervention: confidentiality, informed consent, boundaries, competence, and cultural sensitivity.
  • Explain the importance of avoiding dual relationships and maintaining professional boundaries.
  • Outline strategies for addressing ethical dilemmas that may arise during crisis response, such as conflicting duties of care or breaches of confidentiality.

Cultural Competence in CISM

  • Examine the impact of cultural factors on individuals' experiences of crisis and their help-seeking behaviors.
  • Identify cultural considerations related to communication, beliefs about mental health, and preferred coping strategies.
  • Develop skills for providing culturally responsive crisis intervention services, adapting the approach to meet the unique needs of diverse populations.

Specific CISM Interventions

Defusing

  • Describe the purpose and goals of a defusing session: to mitigate acute stress reactions and provide early support.
  • Outline the key components of a defusing session: introduction, fact phase, thought phase, reaction phase, and teaching phase.
  • Explain how to facilitate a defusing session effectively, creating a safe and supportive environment for individuals to share their experiences and feelings.

Critical Incident Stress Debriefing (CISD)

  • Discuss the purpose and goals of a CISD: to facilitate emotional processing, promote adaptive coping, and prevent long-term psychological sequelae.
  • Outline the steps of a CISD session: introduction, fact phase, thought phase, reaction phase, symptom phase, teaching phase, and re-entry phase.
  • Explain the guidelines for conducting a CISD effectively, including creating a structured and confidential environment.
  • Understand the controversies surrounding CISD and its appropriate use, considering the potential risks and benefits.

Individual Crisis Intervention

  • Describe the principles of individual crisis intervention: active listening, empathy, validation, and problem-solving.
  • Explain how to assess an individual's level of distress and identify their immediate needs.
  • Outline strategies for providing emotional support, reducing anxiety, and promoting coping skills.
  • Detail the application of psychological first aid (PFA) in individual crisis intervention, focusing on providing basic needs and reducing distress.

Crisis Communication Techniques

  • Learn effective verbal and non-verbal communication skills for de-escalating tense situations.
  • Understand the importance of active listening, empathy, and clear messaging during a crisis.
  • Practice techniques for communicating with individuals who are experiencing extreme emotional distress or are resistant to assistance.

Advanced CISM Applications

Working with Specific Populations

  • Discuss the unique needs and challenges of specific populations affected by crisis: children, adolescents, older adults, first responders, and disaster survivors.
  • Outline strategies for adapting CISM interventions to meet the specific needs of these populations, considering their developmental stage, cultural background, and pre-existing vulnerabilities.
  • Understand the importance of collaboration with other professionals and agencies to provide comprehensive support to these populations.

Self-Care for CISM Providers

  • Explain the importance of self-care for CISM providers, given the potential for vicarious trauma and burnout.
  • Outline strategies for managing stress, promoting emotional well-being, and preventing compassion fatigue.
  • Develop a personal self-care plan that includes regular exercise, healthy eating, sufficient sleep, social support, and mindfulness practices.

Complex Trauma and CISM

  • Discuss the unique considerations when applying CISM interventions in cases of complex trauma.
  • Understand the importance of safety and stabilization in working with individuals who have experienced chronic or repeated trauma.
  • Outline strategies for addressing dissociation, emotional dysregulation, and relational difficulties in the context of CISM.
