These electrical signals are what the monitor detects and displays as a rhythm. However, for a pulse to be felt, the heart muscle must physically contract with enough force to eject blood into the arteries. In PEA, the heart muscle either fails to respond to these electrical signals entirely, or the contraction is so weak that it cannot move blood effectively. This disconnect can be caused by problems such as severe blood loss, which leaves the heart with no blood to pump, or physical obstructions that prevent the heart from filling or emptying properly. Even though the electrical blueprint for a heartbeat is present, the mechanical output of the heart is missing, resulting in an organized rhythm on the monitor without a corresponding physical pulse.
